About diagnosis, treatment and recovery
What is GIST and how does it differ from other GI cancers?
GIST arises from cells of Cajal and is usually driven by a KIT/PDGFRA mutation; this makes it sensitive to targeted drugs and gives it a different treatment logic from classic adenocarcinomas.
How is treatment decided and who sets the sequence?
The decision rests with the multidisciplinary oncology council, not a single physician; the surgery-drug sequence is set according to mutation profile, tumor size, mitotic rate and stage.
Why might drugs be given first (neoadjuvant)?
In large or organ-preservation cases, shrinking with imatinib enables less extensive, safer surgery.
What matters most in surgery?
Removing the tumor en bloc with a clean margin (R0) without rupturing its capsule is essential; routine lymph node dissection is not required. In suitable cases a robotic/laparoscopic approach is preferred.
Why is mutation analysis important?
Subtypes such as KIT exon 11/9 and PDGFRA (including D842V) directly affect drug and dose choice; some mutations respond differently to standard imatinib.
How long does post-surgery (adjuvant) drug therapy last?
Duration is set by the council according to risk level and mutation type; protective TKI therapy is planned for high-risk patients.
What is done if GIST recurs or spreads?
In metastatic disease, sequential TKI therapy is used; in suitable cases interventional oncology (ablation, TACE/TARE) may be added for liver metastases. The decision is updated in the council.
Should a small, asymptomatic GIST be operated immediately?
Depending on size, location and risk features, some small lesions may be followed while others require surgery; this too is assessed by the council.
How is follow-up done?
Periodic imaging (usually CT/MRI) and clinical check-ups are performed according to risk level; remote tele-oncology follow-up is possible.
I am coming from abroad, how does the process work?
Your file is evaluated remotely, the council decision and a transparent offer are shared; visa, transfer, accommodation and medical interpreter support are provided.
