TREATMENTS

How it is given and which cancers it treats

Components of chemotherapy
The board decides which drug is given, over how many cycles and with what, according to the tumor type, stage and your general health.

Before surgery, after surgery or on its own

Chemotherapy is given before surgery to shrink the tumor (neoadjuvant), after surgery to lower the risk of return (adjuvant), or to control spread disease. The aim sets the length of treatment and the choice of drugs.

The cycle schedule

The drug is given on set days, followed by a break for the body to recover; a treatment day and its break make one cycle. Cycles usually repeat every two or three weeks. Before each cycle, a blood test and an examination check whether it can be given.

Infusion, port and oral treatment

Most drugs are given intravenously as an outpatient in the chemotherapy unit. For long courses or drugs that irritate veins, a small port is placed under the skin of the chest so the arm vein does not have to be found each cycle. Some drugs are taken at home as tablets.

Side-effect management and supportive care

Nausea, fatigue, hair loss and drops in blood counts depend on the drug; most are eased with precautions taken in advance. Anti-nausea drugs are given routinely with every cycle, and injections that support blood cell production are added when needed. Nutritional support and mouth care are part of treatment.

Together with radiotherapy (chemoradiotherapy)

In rectal, cervical, head and neck and esophageal cancers, chemotherapy is given in the same weeks as radiation; the drug boosts the effect of radiation. The radiotherapy page explains the radiation techniques.

Together with immunotherapy and targeted drugs

In some cancers such as lung, stomach and breast, chemotherapy is given in the same cycle as immunotherapy or drugs aimed at the tumor’s gene change. The tumor’s biomarker tests decide the combination; immunotherapy has its own page.

Heated intraperitoneal chemotherapy (HIPEC)

In selected cancers that have spread to the peritoneum, after the visible tumor is cleared with surgery, the abdomen is washed with heated chemotherapy in the same operation. The HIPEC page explains who it suits and how it is done.

Which cancers
Each cancer's page explains the role of chemotherapy in detail; here you find a short summary.

Breast cancer

In breast cancer, chemotherapy is given before or after surgery depending on the subtype; in triple-negative and HER2-positive tumors it often starts before surgery. The breast cancer page has the details.

Colon cancer

In stage III and selected stage II colon cancer, chemotherapy after surgery lowers the risk of return; in spread disease it is given together with targeted drugs. The colon cancer page has the details.

Lung cancer

In lung cancer, chemotherapy is used before and after surgery, together with radiation or in the same cycle as immunotherapy in advanced stages. The lung cancer page explains the options by stage.

Ovarian cancer

In ovarian cancer, platinum-based chemotherapy is the backbone of treatment together with surgery; in advanced stages chemotherapy may be given first and surgery done after. The ovarian cancer page has the details.
